When Dr. Rafael A. Peñalver practiced medicine in his native Cuba, physicians were considered part of the family. Dr. Peñalver wished to replicate that same feeling and he dreamed about a facility that would provide high-quality, affordable, preventive medicine, education and personalized treatment for all.

In 1996, his dream became a reality. The clinic is designed to be a place of comfort and refuge and is located in a spacious, 22,000-square-foot building. The clinic is easily accessible by public transportation, has free parking, and provides transportation services for the handicapped and elderly.

Your financial evaluation will determine what amount you have to pay. To assist you in qualifying for health benefit programs, please bring the following information:

• Photo ID - drivers license, passport, Florida ID
• Proof that you live in Miami-Dade County - rent receipt, light, water or other utility bill
• Proof of income and family size - last four consecutive pay stubs, most recent income tax, bank statements

Hours of Service
Monday: 8 a.m. to 4:30 p.m.
Tuesday through Friday: 8 a.m. to 8 p.m.
Saturday: 9 a.m. to 1 p.m.
