Jefferson Reaves, Sr. Health Center is a focal point for healthcare services in the Overtown Community. It is committed to the dignity, social well-being and health of the community and the patients who are served at the clinic.

The center offers comprehensive primary care services and, through the participation in the Jackson Health System, facilitates referrals to Jackson Memorial Hospital for specialized services. At the Jefferson Reaves center, staff provides care and treatment in three different languages: English, Spanish and Creole.

Your financial evaluation will determine what amount you have to pay. To assist you in qualifying for health benefit programs, please bring the following information:

• Photo ID - drivers license, passport, Florida ID
• Proof that you live in Miami-Dade County - rent receipt, light, water or other utility bill
• Proof of income and family size - last four consecutive pay stubs, most recent income tax, bank statements

Hours of Service
Monday, Tuesday, Thursday and Friday from 9 a.m. to 5 p.m.
Wednesdays from 10 a.m. to 6:30 p.m.

You can access the Jefferson Reaves, Sr. Health Center from Miami-Dade transit bus number 77 and from the Metrorail via the Culmer (Overtown) Station.
